Thank you for your interest in Hope! The first step in getting paired with a mentor is to complete a Hope packet, which includes a questionnaire, covenant, depression/anxiety test and stress test. Once the packet is returned, we will schedule a time for you to come in and meet with a Hope staff person to discuss your situation in more detail in a confidential, safe environment. Hope packets are available by request at the Hope table located outside the auditorium after any Sunday service, or you may print and complete each of the following:

1. Hope Covenant
2. Hope Questionnaire
3. AMEN Depression and Anxiety Test
4. Stress Test

Please return by mail or leave completed packet at the Hope table on Sunday.
